    How to Apply

    1. Step 1: Offline application — step 1

      Step 1: The interested applicant should visit the office of the District Social Security Officer or the District Child Protection Officer and request a hard copy of the prescribed application form for the Sero Positive Illness Scheme. Step 2: In the application form, the applicant should provide all the mandatory details and attach self-attested copies of all the required documents, including Aadhaar Card, Birth Certificate, Civil Surgeon’s Certificate, and other supporting documents. Step 3: The applicant should submit the duly filled and signed application form along with the supporting documents to the District Social Security Office or District Child Protection Office. Step 4: The applicant should request a receipt or acknowledgement from the concerned authority to whom the application has been submitted for future reference.
