    How to Apply

      Step 1: Initial Reporting and Documentation

      Visit the health facility where sterilization was conducted and report the case of death, failure, or complication to the Medical Superintendent or Health Officer. Begin collecting all required documents including original sterilization certificates, case papers, identity documents, and bank details from relevant sources.

      Complete Part A of the Claim Form cum Medical Certificate with accurate personal details, sterilization operation information, and nature of claim. Submit all medical records and completed Part A to the Chief Medical Officer or equivalent designated officer at district level for medical certification.
